Abstract

Advanced Practice Nurses (APNs) are playing an increasingly significant role in managing chronic diseases in primary care settings. As the global burden of chronic conditions rises, the integration of APNs into healthcare teams has become pivotal in improving patient outcomes and reducing healthcare costs. This paper explores the contributions of APNs in chronic disease management, focusing on their responsibilities, the challenges they face, and the benefits of their involvement. By examining case studies and evidence from diverse healthcare systems, the paper highlights how APNs can enhance patient care, particularly for those living with long-term illnesses. The paper concludes by offering insights into the potential expansion of APNs' roles in primary care and the healthcare system at large.
